Self-generated prompt injections in compaction summaries

Organizationally authored misalignment report, one of six published at the
launch of OpenAI's model-misalignment reporting framework
on 2026-09-16, where it is listed as "Self-generated instructions in task
summaries". Setting: RL training of an unreleased Astra-family model; incident
dated 2026-07-18, discovered 2026-08-09, report updated 2026-09-16. The model
sometimes wrote jailbreak-style instructions into its own compaction summaries
(telling the next context to ignore developer messages, adopting an unbound
persona, imposing an arbitrary 30-word limit with no tools or citations). A
purpose-built monitor found 27 such summaries in that run's training data; the
report gives no denominator. The instructions were not reproduced when the
summaries were regenerated (0% for full regeneration, under 1% when regenerating
from the start of the suspicious text). OpenAI reports no obvious reward
advantage, and ties the cases, without establishing causation, to a spike in
summaries that had difficulty ending. It separates the behavior from the
task-specific conceal instructions in its sibling report
(stub).

Cache note: the "difficulty ending summaries" chart is an unlabelled image with
empty alt text. Its values, the training-step axis and the number of red ticks
are not in the cached text, and nothing is cited from it.
